お電話をお掛けになる際は、お名前と診察券番号をお伝えください。
フォームにてご記入いただいた内容と照合致します。
一回のご相談は10分以内、料金は1,000円となります。 -

数日内にクリニックよりご自身のメールアドレス宛に、
請求内容とお支払いリンクをお送り致しますのでお支払いください。
Telephone Consultation
Holiday and after-hours counseling will be available via telephone.
- This service is available only for patients who have had an in-clinic visit in the past and are registered.
- As this counseling is for after-hours only, refunds will not be available.

Please enter all necessary information on the “Telephone Consultation Form” and submit.

You will recieve an email from the Clinic with the corresponding telephone number.

Please call the noted phone number.
Please provide your name and Patient Registration Number when calling.Your information will be verified via the Telephone Consultation Form. Each consultation will be limited to 10 minutes, at ¥1,000 per call.

Billing
The Clinic will send an email containing an invoice and payment link within a few days. Please remit payment via the payment link.

- 既に受診をされた事のある患者様のみご利用頂けます。初診の方はご利用頂けません。
- クリニックの診療時間内のみご利用頂けます。時間外は電話相談にて承ります。
- オンライン診療は有料です。処方の対応も可能です。
- Zoomのアプリを使用しますので、診療前に予めアプリをダウンロードダウンロードをしてください。

Making an Appointment
for Telemedicine Consultation
Telemedicine consultation during office hours is available for patients who may have issues visiting the Clinic in person.

This service is available only for patients who have had an in-clinic visit in the
past.
This service is not available for first-time patients. -
Telemedicine consultation will only be available during office hours.
After-hours consultation will be via telephone. - Telemedicine consultation is subject to fees. Prescription of medication is possible.
- Because we use Zoom for the telemedicine consultation, please downloaddownload the app before the appointment.

Provide your name, Patient Registration Number and whether or not you have sent in the form.

The telemedicine consultation meeting URL will be sent to your email address.

At the time and date of the appointment, please enter the meeting by clicking on the URL sent to you. Your attending physician will begin the consultation.
